Health Canada has recalled the above slide distributed by Landscape Structures Inc with model numbers 156456 and 172627, as there is a potential fall risk when children move from the ladder to the slide, and when descending the chute.
Although there have been no reported incidents or injuries in Canada, Landscape Structures Inc. has received 16 reports of injuries to children under 8-years old in the United States, including one bruised arm, 14 fractures to arms and legs, one fractured collar bone and one bruised spleen.
Customers are advised to immediately stop children from using the slide and contact Landscape Structures for removal instructions. Customers will have the option of replacing the slide with another piece of playground equipment, receiving a refund, or receiving credit towards a future purchase.

Between January 2006 to December 2011, approximately 136 units of the slides were sold to schools and other facilities with playground equipment in Canada. Please contact school or municipal officials if a playground near you has a recalled slide currently in use.
