Health Canada has recalled this clip-on high chair following 19 reports -- five of which involved injuries -- in the United States, due to chairs falling from different surfaces. The affected chairs have worn (or missing) black plastic spacers between the cross bar and metal vice clamps.

Consumers are advised to stop using the recalled chairs immediately and contact phil&teds® to receive a free repair kit and revised user instructions.

From May 2006, approximately 5,000 units of the recalled chairs were sold at specialty stores across Canada, and 54,000 in the US.
